Service Description

Initial consultation and assessment with Dr. Holzner If client continues with concierge plan, this amount is applied to first month. Provide consultation and assessment for dementia, including comprehensive review of prescriptive, cognitive, functional, and psychosocial history; determine dementia stage and differential diagnosis; conduct/interpret standardized cognitive and behavioral assessments; and deliver clear, evidence-based diagnostic impressions and individualized care and management recommendations.
